How should I prepare for a Sports Physical Visit?

Please make sure to bring any required paperwork to your visit, and review it in advance to make sure you’ve already completed any student, patient, or parent/guardian sections before the visit starts. We’ve added a few commonly used forms below, but your program, school, or sport may require their own unique form. 

1. Print the form required for the sport, school or program, or download one of the commonly used forms below.

2. Fill out any fields designated for the student, patient or parent/guardian to complete. 

3. Bring the partially completed form with you to your visit and your ZoomCare Provider will take care of the rest.
