The fastest and easiest way to release your child's records is to log into your account and download the records yourself. Click here for instructions on how to download your child's records.
If you would like ZoomCare to release your child's records for you, then the process depends on who will be receiving the records:
Sharing with me, the parent: If you would like your child's records sent to the email or mailing address on file in your account, then you can submit your request to our medical records team via a direct message. Click here to send a direct message.
Sharing with another Healthcare Provider: If you would like your child's records released to another healthcare provider, then you can submit your request to our medical records team via a direct message. Click here to send a direct message.
Sharing with Someone Else: Due to state and federal privacy laws such as HIPAA, you will need to complete a release of information form in order to release your child's records to anyone other than another healthcare provider. Note: In certain cases, teenagers age 13-17 may be required to sign a consent form to release their records. If this is necessary, our medical records team will contact you and walk you through that process.
