Your ZoomCare Provider may request that you fill out the ASRS in preparation for your visit. This is most commonly requested of patients who have a history of ADHD. You will receive an email before your scheduled visit to fill out the assessment attached below.
How to complete and share this assessment:
1. Download the ASRS Assessment form
2. Fill out the assessment. You can either use a pdf editor to select your answers digitally or print the document and take a picture of your answers.
3. Respond to the email from "provider@zoomcare.com" with your attached document
