To schedule a referral outside of ZoomCare, follow these simple steps:
1) If you're insured, please contact your insurance company to better understand where you can be seen in-network. If you're uninsured, go to the next step.
2) Then, you can send us an email at firstname.lastname@example.org to let our Patient Care Coordinators know where to send your referral. We will send your referral and any other necessary documents to the facility of your choosing.
