You can view or download your medical records including encounter notes, labs, x-rays, and visit receipts by accessing your account My Visits.
1. Go to zoomcare.com or use the ZoomCare App.
2. Select "My Visits" from the navigation.
3. Log in to your account when prompted.
4. Select the “Provider Note” for the date of your visit. It will include all lab and imaging reports ordered for that encounter.
5. A PDF will open that you can view or save on your electronic device.
You can also submit a request to have zoomcare staff send you a copy of your records directly to you at the email or mailing address on file via direct message. Click here to send a direct message.
