If you have paperwork needing to be completed by your provider for sports physicals, Family Leave of Absence, Worker's Compensation, or Auto Insurance Claims, please email the complete forms to email@example.com. Please be sure to fill out the patient information section before sending it to us, as providers can not complete their portion without this necessary information first. The best format to use when sending forms is a PDF. We are unable to accept a photo of a form.
Please note that if the form has questions that we are unable to complete based on your most recent visit with us, you may be required to schedule another visit. If the form requires a physical evaluation (such as range of motion, gait, etc.), please schedule an in clinic visit. As long as we have all necessary information to complete this paperwork, you can expect to have the completed documents sent back to you in 1-2 days.
